Immigration Medical Exams – USCIS, I-693 Report of Medical Examination and Vaccination Record
Our United States Citizenship and Immigration Service(USCIS) – certified civil surgeon is Dr. Guillermo Sanchez. He has been practicing medicine for 47 years, as a member of our team, he will complete the final component of your exam. As part of your physical we will complete the I-693 Form (Report of Medical Examination and Vaccination Record), screen you for the required infectious diseases, including tuberculosis, and give you any vaccines that are required. These exams are done at our 9601 Katy Freeway Office located in the Memorial City area on the West side of Houston.
First Visit:
When you come in for the initial visit we will review your vaccine history, begin filling out your I-693, draw the required lab samples and give you any vaccines that you may need. It is very important that you bring in all vaccine records and relevant medical records so that we are able to properly access your exact needs.
You will also need to bring with you each visit:
- a valid passport
- resident alien card
- and driver’s license
Second Visit:
During the course of the second visit we will evaluate all laboratory results including the Tuberculosis screening and you will meet with our Civil Surgeon for the final assessment. If laboratory results indicate that additional vaccines are needed, they may be given at this same visit. After meeting with the Civil Surgeon, and he has verified that all immigration requirements have been met, you will receive a sealed copy of the I-693 document that may not be opened and is to be submitted with your application to USCIS. We will also provide you with an unsealed, identical set of documents for your records.
