About the Examination
The Immigration Medical Exam, commonly referred to as an Immigration Physical or I-693 exam must be completed by a physician office certified by U.S. Citizenship and Immigration Services (USCIS). Passport Health Oklahoma Medical Director, Bret Haymore M.D., is designated as a Civil Surgeon by USCIS.
The I-693 Form is used to report examination results and an appointment consists of health questions, brief physical and mental health screening, blood test for tuberculosis (TB), and for applicants 14 years and older a blood test for syphilis and urine test for gonorrhea. Immunizations are also required and are based on your age and vaccination history. Depending on the applicant’s personal medical history and other health factors, additional screening or testing may be required.
Our clinic is able to complete all portions of the exam, including any vaccinations, and blood and urine collection. If all testing is normal, paperwork can typically be completed within 5-7 days. The applicant will receive the original I-693 form in a sealed envelope for delivery to USCIS and a second copy in an unsealed envelope will be provided for personal records.
