The Immigration Medical Exam and I-693 Form
When applying for a green card, an immigration medical exam must be completed through an authorized civil surgeon. All applicants must take a medical exam as the first step in the process because medical conditions, such as Tuberculosis, active syphilis or drug abuse could disqualify an applicant from receiving a green card.

During your scheduled appointment with Passport Health Atlanta, the Civil Surgeon will record the results of the exam on the USCIS Form I-693, Report of Medical Examination and Vaccination Record. Upon review of the I-693 form by the immigration officer the applicant’s admissibility will be determined.
