Immigration Physical Exams – USCIS, I-693 Report of Medical Examination and Vaccination Record
Dr. Michael A. Cromer, our Medical Director, is a USCIS, United States Citizenship and Immigration Service designated civil surgeon. With this designation, we perform Immigration Physicals (Form I-693, Report of Medical Examination, and Vaccination Record) at our Tampa Clinic (located in Carrollwood).
Immigration Physicals are performed most days of the week and consist of 2 visits.
First Visit:
You will need to provide basic information, a passport, sign the required immigration form, and your vaccine history. We conduct the required physical and mental reviews, tests for immune defenses, Tuberculosis exposure, and other communicable diseases required by USCIS. We stock all USCIS required vaccines and can administer them the same day, or during the follow-up visit.
Second Visit:
If necessary, you will return to Passport Health of Tampa Bay for the interpretation of tests performed during the first visit, and to receive any further vaccines, medicine, or testing required to complete the I-693 Form.
Upon completion of all USCIS I-693 Form requirements, you will be given the completed and signed I-693 Report of Medical Examination and Vaccination Records Form in a sealed envelope. This envelope may not be opened and is to be submitted to the USCIS as part of the application process. An unsealed, identical set of documents will also be provided to you at this time.
