The immigration medical examination requires two office visits. First, our Registered Nurse (RN) reviews your medical history, starts the I-693 application, collects any required lab work, and provides vaccinations. The second visit is with the Civil Surgeon for the hands-on medical exam. This two-step process ensures thoroughness and compliance with immigration medical exam requirements.
If there are no complications, you can expect to receive your completed paperwork 10-14 days from your first appointment.
A $75 deposit is required at the time the appointment is scheduled, but is deducted from the total price upon exam completion.

Preparing for your visit – items to bring to your appointment:
- Current vaccination records
- Previous documentation of tuberculosis treatment, if applicable
- Include past chest-x-rays, if applicable
- Photo identification such as a passport or driver’s license
Additional Information:
At Passport Health Colorado, we stock all required vaccinations which can be given on the first or second visit. Once the I-693 is completed it will be given to the applicant in a sealed envelope for delivery to USCIS. A second copy, in an unsealed envelope, will be provided to the applicant for their personal records.
Payment is due at the time of service for all fees, lab work, x-rays, etc.. We accept cash and major credit cards.
