From Chandler - Follow the 202 east and exit at Val Vista. Continue north to Warner Road and make a left. Our Passport Health clinic will be on your right.

From Mesa - Take the 202 South past the US-60 and exit at Higley. Take Higley north to Warner and turn left. Follow Warner past Val Vista, our clinic will be on your right.

From Deer Valley High School - Follow Union Hills Drive west to 79th Avenue. Make a left at 79th, the clinic will be on your left.

From Sun City West - Take the 101 North, exiting at Union Hills Road. Turn right and then right again onto 79th Avenue. The clinic will be on your left.

From the West (via Loop 202 Red Mountain) - Take Loop 202 E toward Mesa. Exit at McKellips Rd via exit 23. Turn left onto E McKellips Rd. Continue for about 1 mile. The clinic is on your right, in Suite 109.

From the East (via E McKellips Rd) - Head west on E McKellips Rd toward N Recker Rd. Continue straight past N Recker Rd. The clinic will be on your left, in Suite 109, shortly after passing the intersection.

Directions -Our clinic is located on 44th Street, just south of the 202. Exit at 44th and turn right at COFCO Drive. Ample parking is available in front of the clinic entrance at the West Wing.

Interior Directions - Take elevator down to Plaza level. Take Plaza North corridor. Turn right at the Y, continue past the restrooms. The suite is on the left at the end of the corridor.

Cross Streets - Located inside the grey building on the corner of 7th Street & Fairmount Ave, a few blocks south of Indian School Rd.

Parking - Enter off of 7th Street and parking will be located in front of the building or in the rear. DO NOT park under covered spots.

Door Entry - Enter from the main building door and take the elevator to the 3rd floor. The clinic will be located on the left.

From Paradise Valley - Follow Hayden Road north to Raintree. Make a right at Raintree and then a right at Northsight. Our clinic is just north of Northsight Park.

From Desert Ridge - Take the 101 East/South and exit at Raintree. Follow Raintree west and turn left at Northsight. Turn right at Northsight Park. The clinic will be on your right.

From the 101 headed south - Turn Right onto Bell Rd. Continue going straight until Parkview Pl. Turn Left onto Parkview Pl. Make a right at the plaza monument sign. Continue going into the parking lot, it is the second building you pass to the left. #100 is written in Bold Black letters.

From the 101 headed north - Turn left onto Bell Rd. Continue going straight until Parkview Pl. Turn Left onto Parkview Pl. Make a right at the plaza monument sign. Continue going into the parking lot, it is the second building you pass to the left. #100 is written in Bold Black letters.

Heading north on the 303 - Turn right on to Bell Rd. Continue to Parkview Pl. Turn right onto Parkview Pl. Make a right at the plaza monument sign. Continue going into the parking lot, it is the second building you pass to the left. #100 is written in Bold Black letters.

Landmarks - Located at Southern and Fairfield, near Shalimar Golf Club and just off the 101, Passport Health's temple location is conveniently located to serve you.

Directions - Turn north on Fairfield, our clinic is located in the second office building on the left. The parking lot has plenty of spaces.
