Passport Health is now Cardinal Health’s chosen provider for vaccine credentialing services as well as destination-specific travel medicine for those who travel internationally for work.

To book an appointment at any of Passport Health’s 300+ clinic locations:

• Call to schedule your appointment.
• When booking, let the scheduler know you are a Cardinal Health employee.
• Have basic information available. The scheduler will ask for the patient name, date of birth and email address, as well as travel dates and destinations when appropriate.
• Once you schedule your appointment, be sure to complete your online medical history. This helps ensure the best quality of care at the time of your appointment.
